Basic Eligibility Requirements

✓Grade Level Standing:
Must be an incoming Grade 7 student (currently graduating Grade 6 elementary pupil or equivalent eligible candidate).

✓Filipino Citizenship:
Filipino citizen with an authentic PSA Birth Certificate issued by the Philippine Statistics Authority.

✓Satisfactory Academic Performance:
General average of at least 80% with no failing grades in any subject or grading period on Form 138 (SF9).

✓Demonstrated Artistic Talent:
Proven aptitude, artistic dedication, and potential in one of the 6 specializations (Dance, Music, Theatre Arts, Visual Arts, Media Arts, or Creative Writing).

✓Health & Boarding Readiness:
Physically and emotionally fit to participate in intensive arts training alongside academic studies, and ready for campus boarding in Bagac, Bataan.

Form 138 (Report Card)
Grade 6 Report Card (SF9) signed by the school principal showing certified grades.
PSA Birth Certificate
Official PSA-issued copy certifying Filipino citizenship, legal name, and birth date.
2x2 ID Photo
Recent colored passport-style photograph with white background and full frontal face view.
Good Moral Certificate
Official certificate signed by the elementary school principal or guidance counselor.
